Comprehensive work up of right ventricular reverse remodelling after successful pulmonary endarterectomy for chronic thrombembolic pulmonary hypertension - a cardiac magnetic resonance study
نویسندگان
چکیده
Background About 3.8% of patients suffering an acute pulmonary embolism will develop chronic thrombembolic pulmonary hypertension (CTEPH). As a consequence of increasing pulmonary pressure and resistance right ventricular systolic and diastolic function deteriorate while right ventricular volumes increase. Pulmonary thrombendarterectomy (PEA) offers a potential cure with excellent longterm outcome. Cardiac MRI is an ideal tool to monitor remodelling and reverse remodelling before and after PEA, because it allows complete volumetric coverage of the complex right ventricular geometry. Several non invasive parameters which characterize diastolic, systolic and ventriculo-arterial properties can be derived from theses measurements. To quantify the remodelling potential of the right ventricle we performed volumetric measurements before and after PEA

Right ventricular reverse remodelling after balloon pulmonary angioplasty.
Balloon pulmonary angioplasty (BPA) has been reported to improve haemodynamics and functional capacity, with an acceptable risk, in patients with chronic thromboembolic pulmonary hypertension (CTEPH) who are not candidates for pulmonary endarterectomy. However, right ventricular (RV) function, an important predictor in CTEPH, remains to be elucidated. We aimed to examine the impact of BPA on RV...
متن کاملRight ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease
Introduction: In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...
متن کاملRight ventricular adaptation to pulmonary pressure load in patients with chronic thromboembolic pulmonary hypertension before and after successful pulmonary endarterectomy - a cardiovascular magnetic resonance study
BACKGROUND The aim of the study was to characterize RV adaptation to varying loading conditions in patients with chronic thromboembolic hypertension (CTEPH) before and after pulmonary endarterectomy (PEA). Nearly 4% of patients with pulmonary embolism develop CTEPH. PEA offers a cure with excellent outcome. By use of cardiovascular magnetic resonance (CMR) combined with hemodynamic measurements...
متن کاملPulmonary endarterectomy normalizes interventricular dyssynchrony and right ventricular systolic wall stress
BACKGROUND Interventricular mechanical dyssynchrony is a characteristic of pulmonary hypertension. We studied the role of right ventricular (RV) wall stress in the recovery of interventricular dyssynchrony, after pulmonary endarterectomy (PEA) in chronic thromboembolic pulmonary hypertension (CTEPH). METHODS In 13 consecutive patients with CTEPH, before and 6 months after pulmonary endarterec...
متن کاملPulmonary Vascular and Right Ventricular Reserve in Patients With Normalized Resting Hemodynamics After Pulmonary Endarterectomy
BACKGROUND Patients with normalized mean pulmonary artery pressure (mPAP) after pulmonary endarterectomy (PEA) for chronic thromboembolic pulmonary hypertension (CTEPH) do not always regain normal exercise capacity. We evaluated right ventricular function, its interaction with both pulsatile and resistive afterload, and the effect of sildenafil during exercise in these patients. METHODS AND R...
